Prescription errors sometimes occur in a pharmacy but they can be prevented. Most of these mistakes include prescribing the incorrect medication, or wrong dosage. These errors can happen for various reasons. It could be due to overworked, lack of communication between the health care professionals, or even when pharmacists do not undergo proper training
There are things that pharmacies can do to avoid these mistakes.
- Having Enough Staff- the demands on pharmaceutical stags are continuously increasing. It is important to have adequate staff to handle the workload to avoid any mistakes from overwork and stress.
- Check the Prescription Entries – pharmacist can use different methods to check on the patient’s record information and history of purchasing such medications which help to reduce mistakes.
- Check the Prescription Attentively -The staffs need to check the written prescriptions properly if it matches the information in the computer system.
- Clarify the Prescription – double-check the prescription, especially for unfamiliar abbreviations and other symbols. It’s better to ask your comrade rather than just take a guess.
- Counseling the Patients – mistakes can be avoided by instructing the patients on how to properly take the medication, verifying that they have the right medication, and taking time to answer some of the questions from the patients. The staff can also open the medication containers for the patient to see which can avoid errors if the medication looks different from what they usually know.
