Appointment Scheduling

• Arrive on time for your scheduled appointment
• Provide at least 24 hours' notice if you need to cancel or reschedule your appointment
• Pay any applicable cancellation fees if you fail to show up for your appointment or cancel with insufficient notice

We reserve the right to charge a cancellation fee for appointments that are missed or cancelled with less than 24 hours' notice. The current cancellation fee is $25.

When you schedule an appointment or use our services, we collect personal information from you, including contact information, health information, and payment information. We use this information to:

• Provide and manage eye care services and appointments
• Process payments and manage our business operations

You waive any and all objections to the exercise of jurisdiction over you by such courts and to venue in such courts.

No waiver by Coop City Pharmacy of any term or condition set out in these Terms shall be deemed a further or continuing waiver of such term or condition or a waiver of any other term or condition, and any failure of Coop City Pharmacy to assert a right or provision under these Terms shall not constitute a waiver of such right or provision.

If any provision of these Terms is held by a court or other tribunal of competent jurisdiction to be invalid, illegal, or unenforceable for any reason, such provision shall be eliminated or limited to the minimum extent such that the remaining provisions of the Terms will continue in full force and effect.

Entire Agreement

These Terms, our Privacy Policy, and any other agreements or policies referenced herein constitute the sole and entire agreement between you and Coop City Pharmacy regarding our Site and supersede all prior and contemporaneous understandings, agreements, representations, and warranties, both written and oral, regarding our Site.
